Words are counted by splitting on whitespace. Sentences are split on . ! ? followed by a space or end of text (abbreviations can nudge the count slightly; every counter has this quirk). Paragraphs are blocks separated by blank lines. Reading time assumes ~238 words per minute (average adult silent reading); speaking time assumes ~130 wpm (a comfortable presentation pace).
